Overview

Community Palliative Care are developing new service for consumers, to provide hygiene and respite services.  

We are seeking feedback from clients receiving care under the Community Palliative Care Service.  This questionnaire is for clients over the age of 18 years. 

We ask that you reflect on your personal experience over the past 7 days when responding to these questions. These questions cover physical well-being, social/family well-being, emotional well-being and functional well-being.  

This questionnaire will take less than 10 minutes and we appreciate you taking the time to complete it.
