Residential Transition Care - Patients and Families Survey

Overview

Residential Transition Care program (RTCP) is about to embark on some significant changes to the medical services at the facility at Zillmere.  We want to hear from you before the changes commence, three months into the new model and then at 6 months.

This is the first survey, prior to the commencement of the new model.

This survey is anonymous and will be independently reviewed by the Innovation and Research Team.
