Overview

Community and Oral Heealth Services in Metro North Health Service, are trialling a phone service for the clients of the Community Residential Transitoin Care Progam.  The purpose of the program is to phone clients once a week for the duration of their care, to provide an  opportunity for social conversation and connection.Community and Oral Health services have a volunteer program that supports social connection within our services. This phone program is an extension of that service. One of our volunteers will be paired with you, and phone you weekly for an informal chat. Our volunteers cannot provide advice of any type, but can engage  in general conversation and are here to provide a weekly catch up with you.

The phone call lasts approximately 15 minutes and is for the duration of your care.In order for us to see how well the program is going, we need to ask some questions. These will be asked again at the end of your time with us, so we can use the information to improve our services. It is a short questionnnaire and your responses will be treated in absolute confidence.
