Overview

LINK Innovation Funding commenced in 2015 with the objective of funding and testing innovative partnership initiatives between Metro North and community  organisations dedicated to providing better integrated patient-centred care, improving admission and discharge parctices, and preventing avoidable admissions and re-admissions.  

Why we are consulting

We are seeking feedback from community members, consumers, community organisations and partners to inform planning for LINK 2016 and to evaluate the first year's program implementation.   We are also seeking ideas on how we could improve the program and how we can build in measures of consumer involvement and co-design, partnership, collaboration and service integration.

What happens next

These results will be fed into our planning for 2016 and the design of an evaluation framework.
