Overview

This survey measures how effective the Residential Aged care District Assessment and Referral (RADAR) service provided by Metro North Hospital and Health Service (MNHHS) is at supporting the care needs of residents of Residential Aged Care Facilities (RACF).  RADAR is a group of doctors and nurses with a mix of geriatric, palliative and emergency skills with the goal to deliver quality care to residents with acute or deteriorating health care needs.  RADAR can visit a resident in their facility or support their presentation to hospital.

Over the past year, RADAR across our hospital sites has changed names and introduced new services. RADAR service is embedded into our hospitals including the Royal Brisbane and Women’s Hospital, The Prince Charles Hospital, Redcliffe Hospital and Caboolture Hospital.

Your feedback will assist us to understand if we are meeting the needs of the resident and the health care team that support the RACF.  We would value your input in reviewing and continually improving the RADAR service.  We leave this survey with you to complete after our service, so you can give your feedback openly.  Your feedback will be kept anonymous.
