Overview

Providing excellent care to people with a life limiting illness and their families and carers is a priority for Metro North Hospital and Health Service (Metro North). We deliver adult palliative care services to our local population and support the care of people from other Hospital and Health Services populations who are referred to Metro North.

Metro North has a long history of supporting the care needs of people who are approaching end of life, their families and carers through:

  • domiciliary community services including personal care, nursing and allied health support in the home
  • consultation liaison services in the acute ward setting
  • specialist palliative medicine outpatient services
  • specialist palliative care teams providing palliative care in dedicated palliative care facilities.

Metro North is committed to delivering excellent palliative care, however we know the Metro North palliative care service system and current models of service delivery cannot meet future demands. A draft Palliative Care Clinical Services Plan 2017-22 has been developed to enable Metro North to deliver high quality evidence based services into the future that better meet service demand. The Plan describes Metro North's commitment to redesigning palliative care services across care settings in the health service over the next five years.

A copy of the draft Plan is attached below and on the first page of the survey. Please review the Plan and submit your feedback by June 30, 2017.
